Hallowed Hill is a "secret" world area in Act 1, and is accessed through the River Passage.
To find the entrance, head northeast from Burrwitch Village Riftgate until you cross the large bridge leading to Burrwitch Estates. As soon as you cross the bridge, turn left down the small steps and double back under the bridge. If the steps are blocked you will have to go through the house nearby. Continue east until you find the unmarked cave entrance.
